Topic: Re-registration of MM state ID card

Aloha, i received the ASA message reporting that my doctor will no longer issue MM cards/prescriptions.  It continued to say that efforts are being made to direct affected patients to new doctors.  I responded to the ASA 10/17/09 ASA email twice and even tried calling the phone number provided.  The email box was full and not accepting any more messages and i have not yet received a response to my email.  I was hoping to learn of a new doctor who will re-register me. 
The thing is that this is new to me.  If i am not able to re-register, does that mean i have to start the process all over again with the paper work and the costs?
I am only looking for a contact name and phone number to re-register myself.

Why not ask your doctor to refer you to another physician?
